- Class 10 th Board Certificate containing Date of Birth (DOB).
- Consolidated Marksheet of MBBS Degree / BHMS Degree.
- M.B.B.S. Degree.
- Certificate regarding training in occupational health/ public health or occupational medicine
- BHMS Degree.
